Your Turn - Working with Stockport NHS Foundation Trust

Julia Walsh and Joanne Conway approached Your Turn in 2011 to help them develop a campaign that would span both community and acute settings to reduce pressure ulcers.

fter much work, several different items were produced for us in one or both settings, all with the aim of reducing incidents and educating the public.

The pressure ulcer prevention passport was to sit across both settings as a patient held record of pressure ulcer treatment and prevention strategies.  At a glance this would tell a healthcare professional where the care of the person was up to.

Working with the community district nursing team and professional carers agencies, the team developed a simple carers checklist using the five most common pressure ulcer risk factors.  Working with the care agencies trainig was provided for all staff and stickers and leaflets were distributed to sit in the service users packs to remind the carers to check for the five triggers.

In addition to these strategies a more in-depth leaflet was also produced to help with non-concordant patients.  This leaflet ahs already had a significant impact, reducing the non-concordant patients to nearly zero in just two months.
